SwimFIT Liability Waiver & Release Agreement

1. Acknowledgement of Risk

I understand that participation in SwimFIT activities—including but not limited to swimming, surfing, fitness training, excursions, motorbike riding, and any other physical or recreational activities—carries inherent risks of injury, illness, or even death. These include, but are not limited to:

• Muscle strain, dehydration, injuries from falls or impacts
  

• Drowning or water-related incidents
  

• Accidents during travel or excursions
  

• Heat-related illness, allergic reactions, or adverse responses to exercise

I voluntarily assume all such risks.

2. Health Declaration

I confirm that:

• I am physically and medically fit to participate in all SwimFIT activities.
  

• I have disclosed any relevant medical conditions, allergies, or injuries.
  

• I will consult my doctor before participating if I am unsure of my health status.
  
  

3. Medical Consent

I authorize SwimFIT staff to arrange emergency medical treatment if deemed necessary. I understand that any medical expenses incurred are my responsibility.

4. Personal Responsibility

I agree to:

• Follow instructions given by SwimFIT staff or coaches.
  

• Behave respectfully and responsibly during all activities.
  

• Wear appropriate gear and stay hydrated.
  

SwimFIT reserves the right to remove me from any activity or retreat without refund if my behavior endangers myself, others, or the quality of the experience.

5. Waiver & Release

In consideration of being allowed to participate in SwimFIT services, I hereby:

• Release SwimFIT, its owners, staff, and affiliates from any and all liability for claims, damages, injuries, or losses, whether physical or psychological, that may result from participation.
  

• Agree not to pursue legal action against SwimFIT for any accident, injury, or loss incurred during participation, unless caused by gross negligence.
  

6. Media Release

I grant SwimFIT permission to use photographs or video footage of me taken during the retreat or training for promotional purposes unless I opt out in writing.

7. Legal Standing

This agreement is binding upon me, my heirs, next of kin, executors, and assigns. It shall be governed by the laws of the Philippines.
